About the role
As our Community Coordinator you'll be a key part of the founding team at Inclusia Living — helping to shape what good supported living looks like in practice. You'll hold a small caseload of people you support, making sure their plans are lived out day-to-day, reviewed regularly, and always centred on what matters to them.
Responsibilities
- Lead day-to-day coordination of support for a small group of people, ensuring plans are followed and regularly reviewed
- Build meaningful daily routines, activities and goals in genuine partnership with the people you support
- Liaise with families, carers, social workers and other professionals to maintain joined-up, high-quality support
- Complete support documentation, risk assessments and outcome records accurately and on time
- Support and mentor junior team members, acting as a first point of escalation
- Contribute to the development of Inclusia's service model as a founding team member
What we're looking for
- Experience in supported living, residential care or a related social care setting
- A genuine commitment to person-centred values and positive risk-taking
- Strong communication skills — written and verbal — with the ability to work professionally with families and commissioners
- Ability to manage multiple priorities calmly and thoughtfully
- NVQ/QCF Level 3 in Health & Social Care (desirable — we will support the right person to achieve this)
- Enhanced DBS check required (we will process this)
